EvaluCare Leadership Faculty


Jason Minor, MS HCA, CHCQM, CLSSMBB, CMQ/OE, CPHQ, CPPS, PMP, founded EvaluCare in 2020 and created CARE-AI after 30 years in quality and regulatory leadership, including serving peer reviewer for Centers of Excellence, System Director of Continuous Systems Improvement at UVM Health's Jeffords Institute for Quality, overseeing continuous quality improvement across a six-hospital system in Vermont and New York.
He currently serves as Board Chair of the Vermont Program for Quality in Health Care. Lecturer in Healthcare Quality at the University of Vermont and serves as a quality peer reviewer for the VCP Centers of Excellence.

​CARE-AI was not designed in a consulting firm or a technology lab. It was designed by someone who has runs and designs quality programs, managed surveys, led clinical improvement projects, and sat in the board room explaining quality performance to trustees. That operational foundation is embedded in every element of the infrastructure.
Jason has led cross continuum of care initiatives, including improving substance use disorder treatment access, state agency strategic planning, facilitated cross organization statewide improvement initiatives.

Catherine “Cathy” Fulton, MS, CPHQ, brings over 30 years of executive leadership in healthcare quality, patient safety, and system transformation across diverse care settings, including behavioral health, community hospitals, and regional trauma systems. She served as Executive Director of the Vermont Program for Quality in Health Care (VPQHC), the state’s independent, legislatively designated quality improvement organization, where she led statewide initiatives focused on patient safety, care coordination, health system performance, and regulatory alignment.
Cathy held senior quality leadership roles, including Quality Systems Manager at Westchester Medical Center, a major regional trauma center, where she led quality and safety infrastructure supporting complex care.

Throughout her career, she has worked at the intersection of policy, practice, and performance improvement, partnering with hospitals, state agencies, and multidisciplinary stakeholders to advance data-driven quality strategies, reduce harm, and strengthen system reliability. Her leadership has supported statewide initiatives in areas such as transitions of care, readmission reduction, patient safety surveillance, infection prevention, and suicide prevention.
